What to Bring to Your First Appointment
To make your first visit as smooth as possible, please bring a valid photo ID, a list of your current medications, and any recent dental X-rays (if available). Don’t forget to complete your new patient forms ahead of time — you can find them below.

Park Hill Smile is a fee-for-service dental practice, which means we focus entirely on providing the highest quality care without the limitations of insurance networks. We accept and file PPO insurance plans for patients. In addition, we offer in house membership plans.
